MONITORING OF INNOVATIONS IN THE SPHERE OF PUBLIC HEALTH CARE MANAGEMENT AND FINANCING IN THE REGIONS OF THE RUSSIAN FEDERATION

In 2005 the Federal database on implementation health care reform in Russian regions has being created on initiative and with support of the WHO Project on Health Policy and Management. In 2006 the database was updated. The Independent Institute for Social Policy (IISP) contributed to this work by creation of database on mechanisms of health care financing and management in Russian regions. The information was gathered by survey among heads of public health bodies of the Russian Federation regions and heads of the MHI Funds. The collected data refer to 2004 and 2005 and include different indicators and qualitative characteristics on separation of power between regional and municipal authorities in the sphere of health care, ways of their activities coordination, budget and insurance financing of health care, medical, economic and financial standards in use, modes of medical services payment, methods of drug prices regulation, etc. The innovations in health care financing and management are indicated for last five years.

The previous year was marked by considerable changes in public health care management and financing: the increase of the role of compulsory health insurance institutions in health financing, the implementation of new model of drug regulation and procurement for targeted categories of population, start of health facility network restructuring according to requirements of the new legislation on delimitation of competence between levels of the state, etc. Besides the innovations forced by federal legislation the initiative changes in mechanisms of health care finance and management were fulfilled in about half of regions.

The institutional changes have continued in 2006. The implementation by government the National Project “Health care” will influence public health finance policy in the regions. Several regions will be chosen until the end of 2006 as pilot ones for transition to so called “one channel model of public health funding” and for transformation of inpatient care payment mechanisms. The collection and analysis of data on these changes would contribute to better understanding of situation in the regions and general trends of health care system evolution.

Updating the database on mechanisms of health care financing and management in Russian regions.

As in two previous years, the questionnaires are to be sent on behalf of the Ministry of Health and Social Development to heads of regional health administration bodies and possibly to heads of regional mandatory health insurance funds.

The choice of this option might conflict with unwillingness of the Ministry to send to regions additional questionnaires to drastically increased accounts that are asked from regions after the start of the National Project.

The questionnaire might be distributed by the Central Institute for Health Care Organization and Information. In this case the questionnaires might be sent only to regional health authorities and it should be added by questions previously asked to regional mandatory health insurance funds. The changes in the questions are minimal to be comparable with data for previous two years.

Updated typologies of regions based on their specific systems of public healthcare financing and management and presentations on the maps.
Analytical review on health care financing and management system transformation on regional level in 2006 and general trends in 2004-2006.
